Common subcutaneous protocols use 200-600mg per dose, 1-3 times per week. A 1500mg vial reconstituted with 10mL bacteriostatic water yields 150mg per mL — so 1mL delivers 150mg, 2mL delivers 300mg. IV protocols at higher doses require clinical setting.
